A Washington state UBI # is required of each applicant.

A 10' wide by 8' deep space is $100 plus 5% of sales to benefit Liberty Road Foundation
Limited corner spots are available for an additional $20
Power is available for an additional $10

I agree to abide by all rules set forth herein. I acknowledge that all items presented to jury committee are representive of work to be exhibited in my display, on above date. I understand that I store my artwork at my own risk.
Hold Harmless Agreement:
Upon signing this application, I agree to hold harmless and release the Northwest Holiday Arts, and all sponsoring organizations, the directors, officers, employees, agents and volunteers from any and all claims, loss or damage arising out of or in conjunction with my participation in Northwest Holiday Arts events.
